Health Forms & Immunizations

All new students must submit an immunization records before registering for classes.

Forms should be submitted via the Immunization Form. You will use your Beloit College username and password to log in.

This checklist item will be listed as complete a few days after ALL required items have been received. 

* All students are automatically enrolled in Beloit College's student health insurance. Domestic students who already have appropriate insurance coverage may out of required college health insurance through this site.
